I setup proxy mail gate and mail caw on my server, I use nat and all emails that come from outside in rspamd mark as they come from local ip of pmg, that’s why emails mark as spam. When I use iredmail instead of mail caw it receive mail from external ip of sender. How can I modify to resolve this issue?

What is the point in having the “proxy mailgate” then? Spam should be fought at the point where it enters your perimeter, on the edge, due to SPF lookup, RBL lookup and such things can only word properly if the public IP of the sending mailserver is properly seen. Otherwise you might get blacklisted by too many bounce mails.
I would suggest you do the antispam configuration on the proxy mail gateway, or have the mails directly sent to mailcow (as it is intended to)
