From a mail configuration perspective can’t see any reason why the email wouldn’t be delivered, nor In found anything being rejected in the logs.

Partially answering my question, followed this https://docs.mailcow.email/troubleshooting/debug-resend-quarantine-notifications/ (more and more amazed by the docs!).
Looking at the output looks like the notification should’ve been sent:
Notifying news@maddler.net: Considering 1 new items in quarantine (policy: hourly)
news@maddler.net: 0 of 1 messages qualify for notification
but nothing in the mailbox, nor in the logs.
I’m a bit lost at this point.

I would assume the ‘notified’ field should get updated to 1 after the notification being sent?
